Sijia Wei, PhD, RN, PHN, is a nurse scientist and health services researcher whose work focuses on improving healthcare quality and equity by advancing care coordination for older adults with complex chronic conditions and disabilities. Her research uses nursing science, social network analysis, complex systems science, organizational science, and large-scale real-world data, including electronic health records and claims, to understand how clinicians work interdependently across settings and over time. She is particularly interested in identifying the social, relational, and organizational processes that contribute to care fragmentation, disparities in outcomes, and healthcare costs among adults with multimorbidity, including those living with heart failure, cirrhosis, and Alzheimer’s disease and related dementias.

Dr. Wei is supported by a National Institute on Aging K99/R00 Pathway to Independence Award for her project, “Identifying Hidden Relational Processes that Fragment Care for Older Adults with ADRD and Multimorbidity: A Social Network Analysis Approach.” A central goal of Dr. Wei’s research is to uncover the underlying structures and relationships within healthcare teams and systems that shape coordination. Her long-term research program seeks to translate these insights into multilevel interventions that strengthen care coordination and improve outcomes for people with complex care needs.

Prior to joining Rutgers, Wei completed postdoctoral training in health services and outcomes research at Northwestern University Feinberg School of Medicine and the Kellogg School of Management, where she conducted interdisciplinary research spanning health services research, clinical medicine, organizational science, and computational social science. She received her PhD in Nursing Science from Duke University School of Nursing.
